What is Redwood Coast PACE?
Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults who:
- Are age 55 and older
- Need a nursing home level of care (the state of California determines/certifies each applicant’s need for a nursing home level of care as part of the enrollment process)
- Can safely live in the community with the help of PACE services, and
- Reside in the Redwood Coast PACE service area — Eureka, McKinleyville, Arcata, Bayside, Blue Lake, Carlotta, Ferndale, Fields Landing, Fortuna, Hydesville, Korbel, Loleta, Rio Dell, Samoa, Scotia, or Trinidad
Redwood Coast PACE has Centers in Arcata (on the Mad River Community Hospital campus), Eureka (on the main HSRC campus) and Fortuna (next to the Gene Lucas Community Center on the McLean Campus). We are licensed through the California Department of Health Care Services and the Centers for Medicare and Medicaid Services (CMS).
