POSITION: ADMINISTRATIVE ASSISTANT/MEDICAL RECORDS - PACE Come join a great team that makes a difference in the lives of our elders on a daily basis! The role of this position is to assist with PACE operations by providing key clerical and administrative support. The Administrative Assistant is normally assigned to particular projects that involve some specialized background or knowledge relevant to the assignment. This individual will work somewhat independently, exercising discretion and judgment and may assist in coordinating a function or providing information for the program. Often interacts with participants and their family caregivers, other departments, and external stakeholders. The Facilities Supervisor is responsible for managing and maintaining the property that comprise the three-building campus of the Humboldt Senior Resource Center in Eureka. This person is responsible for planning, organizing, scheduling, assigning and reviewing maintenance, grounds, custodial work and operating systems; performs a variety of tasks related to the supervision of maintenance/custodial operations and activities; and trains, supervises and evaluates the work of assigned employees. This is a great position for someone who wants to pick up a few hours a week and is interested in making a positive impact in the lives of others. Transports packaged meals as directed to homebound elderly clients. Must have good driving record verified by DMV, current CA driver's license, vehicle in good condition and carry minimum CA liability insurance. JOB SUMMERY: Transportation Program Manager is responsible for the overall operations of transportation services provided to ADH and PACE participants in the Adult Day Center, ADC. Program Manager is responsible for the safe operation and performance of vehicles, as well as the supervision and oversight of staff driving vehicles. Responsible for complying with Title 22 regulations, California Highway Patrol (CHP), CalTrans and other regulatory agencies regarding vehicle operation and maintenance. POSITION: DIRECTOR OF FINANCE The Director of Finance is an important member of the HSRC Management Team. Responsible for the agency's fiscal operations and works with the Executive Director to determine short and long term financial strategies for the agency. Qualifications: Must have a Bachelors in Accounting and (5) years progressively responsible fiscal management experience. Experience to include supervisory responsibilities and working for an organization with a budget of $2million or above. POSITION: ASSISTANT ACTIVITY COORDINATOR ADULT DAY HEALTH, EUREKA This position is responsible for the daily management of program activities for older adults attending a day care program. The AAC works with team members to plan and implement monthly activities including leading and supervising these activities. POSITION: DINING CENTER COORDINATOR - SUB Acts as the Hostess for the senior dining center. Coordinates food service in the dining center and home delivered meals. POSITION: PACE ACCOUNTING SPECIALIST Responsible for billing and tracking reimbursement for PACE participant claims under Medicare and Medi-Cal and all reports due to funding sources. Works collaboratively with other PACE team members regarding PACE enrollees to ensure accurate data regarding services provided for PACE participants. Qualifications: Two years medical billing experience, AS/AA degree in accounting/ business or equivalent training, and proven experience in processing receivables and performing general accounting functions. Familiarity with Microsoft Excel, Word and other accounting software. POSITION: KITCHEN AIDE (REDWOOD COAST PACE AND ADULT DAY HEALTH-ALZHEIMER'S SERVICES) Humboldt Senior Resource Center's Nutrition Department is seeking candidates to work 30 hours per week M-F for approximately 6 hours each day. This position shall be responsible for food handling and food service in accordance with food safety guidelines from the programs. Food Service shall include snack, mid-day hot meal as well as distribution of take home meals. Responsible for kitchen clean-up before and after food service. Must have HS Diploma or equivalent, ServSafe certified. Prior food handling experience preferred along with physical ability to lift 35 lbs., bend and stand for most of the time on the job. POSITION: SOCIAL WORKER - PACE: Be a part of PACE (Program for All-inclusive Care for the Elderly), a new program of the Humboldt Senior Resource Center. The PACE Social Worker is an important member of the PACE Interdisciplinary Team. Under the supervision of the Redwood Coast PACE Center Manager, the Social Worker provides direct social work services to the participants of Redwood Coast PACE including participant assessment, case management, counseling and referral. Qualifications: MSW required, licensed preferred, with at least one year experience working with an elderly population and knowledge of local community services for the elderly and their families. --> POSITION: REDWOOD COAST PACE REGISTERED NURSE(RN) F/T (40/hours week) The PACE (Program for All-inclusive Care for the Elderly) RN - Under the supervision of the Center Manager, is responsible for providing direct and indirect nursing care to assigned Redwood Coast PACE participants. Full time RN position in a fast-paced intensive program managing people 55 and older with complicated medical and psychosocial issues. Work with an interdisciplinary team. Must feel comfortable with problem solving, be self-directed and have solid skills in medication management and triage. Must be willing to do home visits. Works in close coordination with the Redwood Coast PACE Primary Care Providers to manage ongoing care and complex clinical situations and support the operation of the clinic.
