Position: Nurse Care Manager The Humboldt Senior Resource Center is seeking a RN Care Manager (NCM) for the Multi-purpose Senior Services Program (MSSP). The NCM is responsible for conducting in depth assessments/reassessments covering medical, health, and rehabilitation concerns; certifying level of care determinations; performing physical assessments as necessary and interpreting clinical findings; working closely with physicians and other health professionals; developing care plans and implementing and monitoring services. Must have a Bachelor Degree from an accredited school of Nursing and current California RN license and 1/yr. exp working with frail elderly. 25-30 hours week with benefits which includes medical, retirement and life insurance after six months.  Paid sick, vacation and holidays.  Other employee paid insurance benefits available. POSITION: PACE Nurse Practitioner. Become an important member of the management team for PACE (Program of All-inclusive Care for the Elderly). This position offers the opportunity to work in a collaborative interdisciplinary team environment, where the Nurse Practitioner is a leader in the development and implementation of the Primary Care Program for our PACE participants. The Nurse Practitioner works within a collaborative practice agreement with the PACE physician. Together with the other Interdisciplinary Team members will provide medical services and care for PACE participants. Duties include: perform initial client assessments, develop comprehensive, therapeutic plans of care, coordinate services with other medical specialists, oversee management of the PACE clinic, ensure compliance with federal and state reporting requirements, and maintain quality services to ensure client satisfaction. Qualifications: Current RN and NP license, experience with the elderly population and demonstrated ability to work as part of a team. Full time, benefitted, exempt. POSITION: SUBSTITUTE HOME DELIVERED MEALS DRIVER. This is a great position for someone who may be retired, have children in school, has a flexible schedule and is interested in meeting new people while making a positive impact in the lives of others. Transports packaged meals as directed to homebound elderly clients. Must have good driving record verified by DMV, current CA DL, vehicle in good condition and carry minimum CA liability insurance. Substitutes need to be able to lift 50 lbs. and load meals into vehicle. Good communication skills needed and ability to keep accurate records. Mon.-Fri. mornings. Hours will vary due to substitute nature of position. $9.00/hour. No benefits.
