POSITION: HUMAN RESOURCES DIRECTOR.   Join a well-respected non-profit and assume responsibility for employee recruitment, orientation, personnel policies and assist with benefit coordination. POSITION: PACE Quality Assurance Coordinator (QAC). The QAC for PACE (Program of All-inclusive Care for the Elderly) is responsible for developing, implementing and evaluating all activities that contribute toward performance improvement related to PACE program operations.  The QAC works closely with the PACE Medical Director and members of the Interdisciplinary Team to ensure quality service delivery. Qualifications: Current RN license in Calif., demonstrated clinical nursing and quality assessments experience or degree in related field with similar health services experience. POSITION: PACE Nurse Practitioner.  Become an important member of the management team for PACE (Program of All-inclusive Care for the Elderly). This position offers the opportunity to work in a collaborative interdisciplinary team environment, where the Nurse Practitioner is a leader in the development and implementation of the Primary Care Program for our PACE participants. The Nurse Practitioner works within a collaborative practice agreement with the PACE physician. Together with the other Interdisciplinary Team members will provide medical services and care for PACE participants. Duties include: perform initial client assessments, develop comprehensive, therapeutic plans of care, coordinate services with other medical specialists, oversee management of the PACE clinic, ensure compliance with federal and state reporting requirements, and maintain quality services to ensure client satisfaction. Qualifications: Current RN and NP license, experience with the elderly population and demonstrated ability to work as part of a team. Full time, benefitted, exempt. POSITION: PROGRAM MANAGER, Adult Day Health (ADH) and Alzheimer’s Services. The Humboldt Senior Resource Center is seeking an experienced manager for ADH and related dementia services. Work with a dedicated group of staff, in a supportive work environment, who assist frail older adults in a day care program. Responsible for the management of daily operations including supervision of personnel. Other duties include: Participation in audits, budget development, coordinating with local referral agencies and ensuring ADH policies are current. The Program Manager will maintain compliance with Title 22 and other regulations governing ADH. Mon.-Fri. 40/hours/week exempt position. Salary depends on experience. Paid vacation, sick and holidays. Medical 1st of the month after 30 days. Life insurance and retirement after six months. Optional employee paid insurance available. BA or RN with two years experience with older adults along with demonstrated management experience. POSITION: SUBSTITUTE HOME DELIVERED MEALS DRIVER. This is a great position for someone who may be retired, have children in school, has a flexible schedule and is interested in meeting new people while making a positive impact in the lives of others. Transports packaged meals as directed to homebound elderly clients. Must have good driving record verified by DMV, current CDL, vehicle in good condition and carry minimum CA liability insurance. Substitutes need to be able to lift 50 lbs. and load meals into vehicle. Good communication skills needed and ability to keep accurate records. Mon.-Fri. mornings. Hours will vary due to substitute nature of position. $8.99/hour. No benefits.
