POSITION: SOCIAL WORKER - PACE: Be a part of PACE (Program for All-inclusive Care for the Elderly), a new program of the Humboldt Senior Resource Center. The PACE Social Worker is an important member of the PACE Interdisciplinary Team. Under the supervision of the Redwood Coast PACE Center Manager, the Social Worker provides direct social work services to the participants of Redwood Coast PACE including participant assessment, case management, counseling and referral. Qualifications: MSW required, licensed preferred, with at least one year experience working with an elderly population and knowledge of local community services for the elderly and their families. Must have proficient computer skills. 40 hrs./week. Mon.-Fri. Excellent benefit package. POSITION: HOME DELIVERED MEALS DRIVER - (Fortuna Area): Transport packaged meals as appropriate to homebound elderly within Eureka, Arcata, McKinleyville, or Fortuna area following food safety guidelines. Deliver hot and/or frozen meals in compliance with policies and procedures and food safety standards. Ensure meal quantity being transported meets requirements of route. Ensure insulated carriers are prepared to hold food at required food safe temperatures. Maintain all meal transport equipment according to department policies and procedures. POSITION: RECEPTIONIST/CENTER COORDINATOR - PACE This position is under the supervision of the Center Manager, the Receptionist is responsible for answering and transferring incoming calls and shall respond to all calls from PACE participants, their family members, providers, Redwood Coast staff and the general public in a courteous, helpful, and professional manner. Manage the flow of visitors coming in and out of the PACE Center, with special attention to participant safety and offering reassurance when necessary. Helps facilitate care coordination and service delivery by generating reports and scheduling internal/external services/appointments. Must have good driving record verified by DMV, current CA driver's license, vehicle in good condition and carry minimum CA liability insurance. This position will be under general supervision of Finance Manager, provides a variety of record keeping and accounting duties for the PACE program according to departmental, professional and regulatory standards. In a team framework, works closely with the PACE Accounting Specialist to ensure that all PACE program accounting and billing is accurately billed, paid and recorded into the HSRC systems. POSITION: REDWOOD COAST PACE REGISTERED NURSE(RN) F/T (40/hours week) The PACE (Program for All-inclusive Care for the Elderly) RN - Under the supervision of the Center Manager, is responsible for providing direct and indirect nursing care to assigned Redwood Coast PACE participants. Full time RN position in a fast-paced intensive program managing people 55 and older with complicated medical and psychosocial issues. Work with an interdisciplinary team. Must feel comfortable with problem solving, be self-directed and have solid skills in medication management and triage. Must be willing to do home visits. Works in close coordination with the Redwood Coast PACE Primary Care Providers to manage ongoing care and complex clinical situations and support the operation of the clinic. Requires valid CA RN license and CPR certification. Benefits include: Paid vacation, sick leave and holidays. Optional employee paid insurance: dental/vision and Aflac. Must possess proven leadership ability and problem solving capabilities. POSITION: SUBSTITUTE FOOD SERVICE AIDE Responsible for food handling and clean up at various HSRC dining sites including Eureka, Fortuna and Arcata. Clean, wash, and prepare food service equipment and home delivered meals carriers. Assume other duties in the preparation and serving of meals under the direction of the Food Service Manager. Must have previous kitchen experience, valid Food Safety Manager Certificate, Food Handlers card or ServSafe certification. Ability to stand for extended time and lift up to 40 lbs. M-F 7am to 1:30pm with some variable hours at times. Subs can work up to five days/week in our busy central kitchen or other sites. Positive work environment preparing meals for older adults.
